On page 10 Grace states her full name is Grace Miller. On page 18, the Doctor calls Maggie “Miss Lee”, indicating that Maggie’s not there under Grace’s name. But here the Church assassins have Grace’s picture. Was Maggie there under Grace’s name, but the doctor in on the insurance fraud? Or did something else screw up the Church’s intel?
